中文版OQ-45.2信效度研究及初步应用

        随着心理咨询行业的迅速发展,心理咨询的质量已成为咨询心理学领域研究的一个重要内容,不少研究者利用标准化量表对来访者咨询效果进行动态评估和监测。OQ-45.2(Outcome Questionnaire,OQ-45.2)由美国杨百翰大学Lambert教授主持开发,是目前广泛应用于国外心理咨询实践的效果评估工具之一。本研究引进了OQ-45.2,在部分修订的基础上,对该量表中文版的信效度进行了检验,建立了咨询效果评估标准及标准化实施程序,并对心理咨询后来访者症状改善的变化趋势进行初步探讨。研究共收集江浙某医院心理咨询门诊和私立心理咨询室来访者共536名,浙某社区居民230名,江、浙、皖三省大学生1759名的评估数据,用于OQ-45.2中文版的信效度验证和咨询效果评估标准的建构;选取浙江某医院心理门诊和私立心理咨询室41例有完整咨询评估记录的来访者数据用于咨询后症状变化趋势的初步研究。对收集数据进行描述性统计分析、相关分析、差异检验和验证性因素分析,得到结果如下:(1)修订后的OQ-45.2理论结构符合原量表的三因素模型,具有良好的信效度,可以广泛应用于不同领域的心理咨询效果评估。(2)根据Jacobson等提出的临床显著性变化评估理论,明确了划界分和可信变化指数(RCI),结果表明,中文版OQ-45.2的总量表和困扰症状、人际关系、社会角色绩效子量表的划界分分别为61/62、33/34、14/15、14/15,RCI分别为17、11、5、4;划界分与原量表指标接近,RCI与原量表指标有差别。基于上述指标,建构了应用于实践的中文版OQ-45.2咨询效果评价标准(分为恢复、改善、无变化和恶化四类)及标准化评估程序,结果发现借助计算机施测,能提高咨询效果应用信息的准确性。(3)使用中文版OQ-45.2对41名来访者(平均咨询3±2次)进行咨询后症状改善的变化趋势探讨,结果提示,来访者症状困扰先于人际关系、社会角色绩效发生改善,结果支持Howard三阶段理论的观点。总之,中文版OQ-45.2具有较好的信效度,建立的评价标准及标准化实施程序有助于今后心理咨询效果评估的研究及实践应用。
    With the rapid development of the field of counseling psychology, the quality of psychological counseling is becoming an important element of the psychology study. Some researchers have already used standardized scales to monitor outcome. Outcome Questionnaire (OQ-45.2) is developed by Professor Lambert M.J. from Brigham Young University of U.S.A., now it is one of the widely used outcome measure tools. Based on introducing the OQ-45.2 and partially revised, this study inspects the reliability and validity of the OQ-45.2, establishes the standard of the outcome evaluation and standardized implementation procedure, it also tastes the initial discussion for the symptom trend of the clients after counseling.The sample consists of 536 counseling center clients from Jiangsu and Zhejiang Provinces, 230 community members from Zhejiang Province and 1759 university students from Jiangsu, Zhejiang and Anhui Provinces. It is used to inspect the reliability and validity of the Chinese version OQ-45.2 and establish the standard of outcome evaluation and standardized implementation procedure. Furthermore, 41 clients of the sample who have integrated counseling record are used to study the symptom trend after counseling.The results as following:(1)The revised OQ-45.2 theory structure corresponds with the Three Factor Model proposed by Lambert, it has high reliability and good validity, and can be widely used to measure outcome in psychological counseling agencies.(2) According to the Clinical Significance concept proposed by Jacobson et al, the cutoff for the Chinese version of OQ-45.2 total scale and symptoms distress, interpersonal relationships, social role subscales are 61/62, 33/34, 14/15 ,14/15 respectively, RCI are 17,11,5,4, respectively. Compare the indices to the English version of OQ-45.2, the two cutoffs are similar but there’s some difference between the two RCIs . Then evaluating criterion which classifies clients into recovered, improved, no changed as well as deteriorated, and standardized implementation procedure were developed. We also found that assisted with computer can effectively improve accuracy of outcome information.(3) Chinese version of OQ-45.2 studies the symptom trend after counseling for 41 clients who had 3 counseling sessions on average. It indicates that clients’symptom distress got improved before interpersonal relations and social role performance; this is corresponding with the three phase model by Howard.In conclusion, Chinese version OQ-45.2 has high reliability and good validity. The evaluating criterion and standardized implementation procedure it builds could help the outcome research and it’s practical application.
